    • Itemized Medical Bills
    • Proof of Payment (receipts)
    • Medical Records/Doctor's Notes
    • Prescription Receipts
    • Copy of Insurance ID Card

    Important Information

    • Submit claims within 90 days of treatment for fastest processing.
    • Ensure all documents are legible and include your policy number.
    • Digital submissions through the portal are processed faster.
